Responsibilities

About the Role

As a Nurse Practitioner, you'll conduct in-depth health assessments of members in their homes or care facilities. These comprehensive evaluations provide critical insights into their health status and care needs. Your responsibilities will include:

  • Performing adult and geriatric health assessments covering medical history, diagnosis, treatment, education, and referrals.
  • Administering point-of-care and lab testing as necessary.
  • Coordinating care with physicians, case managers, and family members.
  • Following up with patients and their Primary Care Physicians as needed.
  • Delivering services at members' residences or Skilled Nursing Facilities.

Qualifications

  • Master's Degree or equivalent experience and successful NP licensure completion.
  • Current NP licensure with prescriptive authority in your practice state.
  • Board certification in a relevant specialty (AANP, AACN, or ANCC).
  • Current certification in BLS, ACLS, or CPR.
  • 1 year of NP experience preferred; new graduates encouraged to apply.
  • Valid driver's license and automobile insurance as per state requirements.
  • Proficient computer skills and familiarity with medical record software.
  • Exceptional verbal and written communication abilities.
