Our Social Service Assistant establishes and promotes the vision and direction of the Department of Social Services consistent with state and federal mandates, and the County's strategic plan as approved by the Board of Supervisors; establishes and implements departmental goals and objectives; establishes appropriate administrative policies, services, structures, controls, and reporting systems for the efficient and effective performance of social service, income maintenance, employment training, and related functions; determines types and levels of services to be provided and standards by which delivery will be evaluated.
The ideal candidate will:
Direct, oversee and evaluate all social service aspects of the facility and each residents behavior on a case-by-case basis.
Identify problems, find solutions, and work together with the facility team to implement them.
Assist with resident needs/requests in addition to care planning and MDS assessments.
Schedule discharges/insurance discharges.
Have previous skilled nursing home experience.
Be responsible and assure that the highest degree of quality care is provided at all times
Collaborate with other departments and professionals to streamline operations
Experience with PCC (of similar EMR experience)
Qualifications:
Bachelor's degree in social work or related field.
Certified as required by the state of employment.
One year of social work experience in a long-term care environment.
One-year supervisory experience in a setting service setting.
