The PHP Nurse Practitioners have a passion for delivering quality care to seniors and other residents in settings that can include skilled nursing facilities, assisted living, independent living communities, and via telehealth. This full-time position will be split between Lincoln Healthcare Center (Hamlin, WV) and Wayne Healthcare Center (Wayne, WV). Our Nurse Practitioners work first shift, Monday Friday. There is never on-call and no weekends or holiday coverage.

Job Summary: The Personalized Health Partners (PHP) NP provides high-quality primary care in a high touch, person-centered care model. The NP diagnoses, treats, and manages acute episodic and chronic illness through wellness visits, early detection of change, and timely treatment. The NP collaborates with PCPs and interdisciplinary teams in medical management of their assigned population and involves residents and their families in developing plans of care. This model reduces hospital admissions and ER visits and fosters a culture focused on customer experience and satisfaction.

Job Duties & Responsibilities
  • Assess, diagnose, and manage chronic and acute conditions.
  • Provide health education.
  • Develop treatment plans and communicate them clearly to residents and other providers, caregivers, etc.
  • Order and interpret lab and diagnostic testing and adjust treatment plans accordingly.
  • Maintain compliant documentation within the EMR system.
  • Collaborate with local and regional operations and clinical leadership to achieve goals.
  • Support strategic initiatives regarding value-based care.
  • Provide an excellent customer experience to foster high satisfaction and retention.
  • Practice ethically and in accordance with the Scope and Standards of Practice and Board Certification.
  • Follow all state and federal regulations, guidelines, and laws.
  • Collaborate with PCP and interdisciplinary care team at the facility.
  • Coordinate care with other providers and specialists, including telemedicine providers.
  • Complete all regulatory required visits (monthly, quarterly, and/or annual) and acute visits as needed.
  • Participate in family conferences and provide advanced care planning and education when needed.
  • Address pharmacy recommendations or pharmacist communications regarding residents.
  • Provide dedicated face-to-face coverage in assigned facilities during work hours to meet resident care and facility needs.
Qualifications & Experience

Required:

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • National certification (AANP or ANCC)
  • Active, unrestricted state license
  • Active DEA license
  • US work authorization
  • 2+ years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult, or family practice
  • Ability to work in a flexible environment and schedule
  • Passionate about providing healthcare to senior and low-income populations

Preferred:

  • Training or clinical experience in geriatrics
  • 3+ years clinical experience as a Nurse Practitioner
  • Experience with coding, ICD-10 and HCC
  • Experience or training in metrics-driven healthcare models
  • Openness to professional development and attending team meetings
  • Experience working in value-based care models
  • Geriatric, assisted living, and/or skilled nursing setting experience
  • Experience working in collaborative healthcare settings to drive positive outcomes
  • Outpatient primary care setting experience
Knowledge/Skills/Abilities
  • Strong oral and written communication skills
  • Detail-oriented with ability to work independently and in a team
  • Customer service mindset with residents and teams
  • Excellent time management; ability to prioritize and meet deadlines
