Salary - $35,000 - $39,375

Position Summary:
The Targeted Case Manager is responsible for providing case management services to assigned consumers to support their overall well-being and progress toward recovery goals. This includes coordinating resources, maintaining documentation, and ensuring adherence to all Pathways policies and procedures.

Educational Requirements:

  • Bachelor's Degree in Human Services or a related field
  • Minimum of one (1) year of experience in Human Services following graduation
  • Equivalent experience in a related field may be considered
Essential Duties and Responsibilities:
  • Provide case management services to assigned consumers based on individualized service plans
  • Coordinate and link clients to appropriate community resources and services
  • Monitor client needs and progress, updating service plans as appropriate
  • Maintain timely and accurate documentation using electronic health record systems
  • Participate in staff meetings, training sessions, and interdisciplinary team discussions
  • Ensure adherence to professional standards, ethics, and confidentiality requirements
  • Perform other duties as assigned
Skills and Knowledge Required:
  • Experience in a behavioral health setting is a plus
  • Competency in using computers and electronic devices, including electronic health records
  • Strong organizational, communication, and time management skills
  • Ability to work independently and collaboratively with diverse populations
  • Knowledge of local community resources is beneficial
Preferred Qualifications:
  • Prior case management or social services experience
  • Familiarity with behavioral health systems, services, and terminology
