Maintain confidentiality and comply with company, state, federal and HIPAA rules and regulations. Charting for insurance purposes. Maintain a positive, professional attitude toward residents, staff and volunteers. Create a safe and healthy environment for clients. Have a sincere commitment to client's recovery. Transport clients to appointments when necessary. Monitor client's emotional well-being and behavior. Connect client with family and community resources. Connect client with legal entities when necessary. Motivate clients toward goal achievement. Meet with clients weekly. Help residents with long-term care plan development and educational resources Key Experience and Education Needed:Bachelor's degree in social work or human services field required 1 year post grad work experience or Master's Degree required.
